This is a required firm that lets us file our pets and owners into our client management system so that we can effectively and efficiently provide the service you and your pet deserve.
Your pet is important to us and this form helps us get to know your buddy’s habits and regular care so that we may make every effort to make your pet’s visit as pleasant and memorable as possible.

Please email the form along with the reason for your visit to info@pawlaciopets.com (Grooming, Daycare, etc)
If your pet is sick or in need of urgent care, please call your pets veterinarian. Thank you!
If you are unable to download and/or email the forms, please come in 20 minutes ahead of your pet’s appointment to complete the forms.

Why is this important?

Our daycare and grooming facility is regulated by The NYC Department of Health which requires all dogs in attendance to be vaccinated for rabies, distemper, parvo, parainfluenza and bordertella.
The City of New York Department of Health and Mental Hygiene Office of Veterinary Public Health Services requires establishments offering daycare and grooming dogs shall require dog owners to provide proof that serviced dog is current on vaccinations providing immunity against rabies, distemper, hepatitis, para influenza, parvo and Bordetella.
